Service Option 1
Services, items, and activities included in basic services and rates are listed herein.

The basic services are those services required by the Washington State law under WAC 388-76, which requires that the home provide basic services to all residents of Cartier’s Adult Family Home whether Medicaid or private pay.
​
Below are accommodations and services for individuals who are developmentally disabled, mentally ill, frail elderly, have dementia or need assistance meeting their needs and activities of daily living. These accommodations and activities are described as basic services, 1-12, below. Cartier's Adult Family Home (AFH):

Provides care and services that actively supports, maintains, or improve each resident's quality of life; and actively supports the safety of each resident.

Reasonably accommodates each resident's individual needs and preferences except when the accommodation endangers the health or safety of the individual or another resident.

Provides reasonable access to a non-pay telephone in an area that affords privacy to the resident.

Processes each resident's laundry weekly or more often as needed. This includes weekly laundering of sheets and pillowcases. Other bedding such as blankets and comforters will be laundered as needed. Facility linen and bedding will always be laundered after any resident leaves the facility. Each resident’s laundry will be laundered separately.

Monitors residents and immediately notify their family, legal representative, and persons identified in the negotiated care plan whenever there is a significant change in the resident’s condition.

Provides assistance with personal care needs such as bathing, grooming, and dressing as needed. Note: haircuts, beard trims, pedicures, manicures, and total or full care are not covered under basic services.

Provides soap, deodorant, lotions, and paper products such as toilet tissue, paper towels, and Kleenex. Note: specialty deodorant, cologne, perfume, designer soaps and shampoos; mouth wash; and dental floss are not provided under basic services.

Provides three nutritious meals daily, and provides snacks between meals and in the evening. Special dietary needs are accommodated. Individual food preferences are taken into consideration to the extent possible. Modified diets are served when ordered in writing by a physician.

Plans an activity program, as specified in residents’ Negotiated Care Plans, and are designed to meet the residents’ preferences. Activities offered may include:

Cable TV viewing

Cartier’s Adult Family Home sponsored “All Resident” day trips or outings, e.g., to Ocean Shores, to Leavenworth, lunch or dinner outing, trips to the local parks, etc.

Dining out: Home-sponsored “all resident” dining out occasional events will be paid for by the home. When not covered by the home, residents will be advised of the need to pay for their meals prior to the event, if they want to participate.

Provides assistance with medications and medical needs as identified in the assessment and care plan within the scope of Cartier’s AFH. Medications will be kept in locked storage. Reminding, cueing of residents to self-administer medications will be provided, as needed.

Verbally communicates with residents' health providers involved in their care, as needed.

Seeks appropriate professionals to provide needed services to the residents. The home will assist residents in obtaining additional on-site health care services, if available and requested by the residents or as ordered by the residents’ physicians.
The basic service rate is negotiable for private pay residents. However, the basic rate is predetermined for residents/clients placed through the Department of Social and Health Services (DSHS) Medicaid. This rate is based on the level of required care and/or selected services and includes the services, items, and activities listed herein. For Medicaid, the level of care and required services are determined initially by DSHS. During the initial phase of placement and later as residents' conditions change, DSHS makes the determination of the level of care in concert with the adult family home staff, family, the resident's legal representative, if applicable, and in consultation with appropriate health care professionals as needed.

Medical or pharmaceutical co-pay for resident and cost of annual Assessment and Negotiated Care Plan must be paid for directly by the resident or guardian/POA if not covered by DSHS.

Except in case of an emergency, Cartier’s Adult Family Home will give the resident a 30-day advance written notice of any changes in the availability of services or charges for services, items, or activities.
